Assessment approaches for hemiplegic shoulder pain – A scoping review

Abstract

Introduction
Hemiplegic Shoulder Pain (HSP) refers to pain felt in and around the shoulder joint in patients who have hemiplegia post stroke. Due to the unknown pathology and understanding of the mechanism, it is difficult to complete a well-informed and appropriate treatment pathway; this may be improved by a thorough assessment process. Overall, there is limited literature to show what current assessments are being utilised in examining HSP and the effectiveness of them in managing this condition.
Aim
The main aim of this scoping review was to gather all potential assessment approaches available for adults with HSP and to identify how frequently each assessment approach occurred.
Method
Each researcher conducted a search in one of the databases that were selected which were: Embase, CINAHL plus, Medline, Cochrane library and AMED. All outcome measures/assessments that were used in relation to HSP were extracted.
Results
A total of 122 out of 585 studies were found that met the inclusion criteria and 43 assessment methods of HSP were identified. The Visual Analogue Scale (VAS) was the most common which occurred a total of 66 times. The next 4 most frequently occurring outcome measures used were, Passive Range of Motion, Fugl-Meyer Assessment, Modified Ashworth Scale and Numerical Rating Scale.
Conclusion
A fully comprehensive assessment that considers different aspects of pain such as severity and timing including functioning and the psychological burden is needed in this area of practice to be able to guide appropriate treatment and decrease the delay in discharge.
