Evaluating the fidelity of implementation of a voluntary sector-led, community-based diabetes prevention and management programme

Abstract

Aim: To assess the fidelity of implementation of a community-based diabetes prevention and management programme (“Living Well, Taking Control”, LWTC).

Background: LWTC is a group-based education intervention promoting healthy lifestyle changes for people with ‘pre-diabetes’ and newly-diagnosed type-2 diabetes. This fidelity study is part of a wider service evaluation of LWTC, and is critical to ensuring successful translation of evidence-based public health interventions into practice.

Methodology: The study was conducted across four facilities in Devon, where the programme was delivered by the voluntary-sector organisation, Westbank. The primary assessment used a fidelity checklist to analyse audio recordings of all intervention sessions conducted from 20th January to 5th March 2015. The checklist was developed to rate levels of implementation, mainly adherence to protocol and competence in delivery. The goal for an acceptable level of implementation was set at 80%. Two independent raters were recruited to test for inter-rater agreement, using Kappa statistics (κ). Audio recorded data was supplemented by course satisfaction data from participant feedback questionnaires, as a measure of facilitator competence.
