Preliminary exploration of the validity of a patient-reported experience measure for eye care services

Abstract

Purpose
The measurement of patients’ experiences and perceptions is central to the development and improvement of patientcentred healthcare and informing decisions about how best health and care resources may be utilised for patient benefit.
Patient Reported Outcome Measures (PROMs) and Patient Reported Experience Measures (PREMs) are used to inform
decisions relating to changes in services. However, a validated PREM tool does not currently exist for eye care services. This
work aimed to explore the psychometric properties of the Eye Care PREM as part of its continuing development.
Methods
Patient experiences of community-based optometric services were assessed using the Eye Care PREM. Responses to the
PREM were evaluated using statistical analyses which followed conventional psychometric assessment using the principles of
classic test theory.
Results
PREM responses from 466 individuals were assessed. The majority of questions had a low level of missing data (<10%). Good
internal consistency was demonstrated (Cronbach’s alpha 0.81) and was improved (0.88) by excluding two questions on
appointment scheduling. Exploratory factor analysis indicated two underlying factors, communication, and care, giving a
Cronbach’s alpha value of 0.84.
Conclusions
Our data provide evidence for the reliability and validity of the Eye Care PREM questionnaire. It is appropriate for use with
individuals attending eye care services. As a result of the current analysis, the Eye Care PREM was modified to provide
consistent scale measures to support further psychometric validation using Rasch analysis in a prospective study.
