Factors associated with acute urinary retention in thrombolysed acute stroke patients - A retrospective cohort study

Abstract
Background: Acute Urinary Retention (AUR) is common after stroke.
Aim: This study tried to identify factors that may predispose acute ischaemic stroke patients to develop AUR post thrombolysis. A secondary aim was to evaluate whether any factors influenced time from stroke onset to AUR diagnosis.
Results: 23.1 % of thrombolysed patients were diagnosed as having AUR. Age (OR 1.07) and hypertension (OR 2.59) were associated with higher risk of developing AUR, with patients with increased mRS at less risk of AUR than patients with mRS of 0. Patients with history of Benign Prostate Hyperplasia (BPH) or who received labetalol pre, during or post thrombolysis for acutely high blood pressure were diagnosed earlier with AUR, as well as patients with TACS, PACS and POCS. Patients who presented with a degree of aphasia (NIHSS item 9 score 1,2 or 3) were diagnosed significantly later with AUR compared to non-aphasic patients, however, this did not result in larger bladder volumes in this group.
Conclusions: This study suggests that previously unknown factors may play a pa rt in increasing a stroke patient’s risk of developing AUR (hypertension/age) in the acute phase. Patients with any degree of aphasia took longer to be diagnosed with AUR, while patients with BPH or who had labetalol to treat raised BP were diagnosed earlier.
