Abstract
Hemiplegic shoulder pain (HSP) is one of the four most common medical complications after a stroke alongside depression, falls urinary tract infections. HSP has a reported incidence of between 30% (acute) and 65% (chronic) in people with stroke. The prevalence of HSP was reported to be higher in the chronic stages rather than the acute stages after stroke. Recent reviews have reported several factors are associated with HSP including glenohumeral subluxation, spasticity, poor motor function, reduced passive range of movement at the shoulder flexion and shoulder and abduction. Several musculoskeletal diagnoses have been proposed as causes of HSP including rotator cuff tendonitis or tears, sub acromial bursitis, bicipital tendonitis. However, few musculoskeletal specialist tests including modified Neer tests (impingement), hand-behind-head (function test) to localise tissue damage, have been tested in people with stroke. A stratified approach to management of HSP was recently published suggesting an integrated approach to HSP guides more effective management in this heterogeneous clinical presentation. A range of treatment options are identified in the literature that will be discussed / highlighted and practiced.
The aims of this workshop are: 1) To explore different assessment approaches for HSP pain that can be applied in clinical practise, referencing musculoskeletal examination techniques that are applicable to the neurological patient 2) To explore and discuss a range of treatment approaches for the hemiplegic shoulder from the acute to the outpatient/community setting 3) To provide opportunities to practise the assessment and treatment approaches
